ABSTRACT

TREATMENT IN THE COMMUNITY FOR PATIENTS WITH SCHIZOPHRENIA

The main aspects of the therapeutic strategy of community psychiatry are being revised.  Its main objective is to achieve the patient’s autonomy within the community.  Nevertheless, considering the aspects of the mental process of the psychotic patient, this new therapeutic relationship can undergo the same difficulties of the chronic condition and alienation criticized by the psychiatric community.  The isolation, the chronic condition and incapacity related to the mental disease do not disappear with the critics and improvement of the psychiatric institutionalization but they form part of the personal and social process of the psychosis.  The difficulties of functioning and social integration can appear again in the community and emerge under new forms.  The therapeutic work with psychotic patients is enormously complicated and in these pages we wanted to share some of our experiences.

RESUMEN

Se revisan las variables esenciales de la estrategia terapéutica de la psiquiatría comunitaria, cuyo objetivo último es el logro de la autonomía del paciente en la comunidad. Sin embargo, dadas las características del funcionamiento mental del paciente psicótico, esta nueva relación terapéutica puede sufrir las mismas dificultades de cronificación y alienación, criticadas por la psiquiatría comunitaria. El aislamiento, la cronicidad y la discapacidad asociada a la enfermedad mental no desaparecen,  con la crítica y la superación de la institucionalización psiquiátrica, sino que forma parte del proceso personal y social de la psicosis. Las dificultades de funcionamiento e integración social pueden reaparecer en la comunidad y emerger bajo nuevas formas. La tarea terapéutica con pacientes psicóticos es de una gran complejidad, en estas páginas hemos querido compartir algunas de nuestras experiencias.
